You acknowledge that you have voluntarily agreed to engage and otherwise participate in Martial Arts practice/training at "The Kodokan" located at 1C - Commonwealth Buildings, Woolwich Church Street - London, Se18 5NS.
You confirm that you understand that this type of sport/training/practice is inherently dangerous, regardless of the level of supervision. Injuries may occur due to physical situations including a slippery floor, an overzealous partner, or a combination of any number of circumstances. It is clear to you that your voluntary participation in this activity will expose you to risk of injury (of any kind), and that any amount of time spent engaging in this activity only makes it more likely that as in any sport, injury man, and is even likely to occur at some time.
As consideration for being permitted to engage in these activities/sport/training and to use the facility provide by "The Kodokan," you hereby agree that you, your assignees, heirs, distributors, guardians, and legal representatives will not make a demand, a claim against, sue, or attack the property of "The Kodokan," its instructors, and/or its authorized agents, and/or its owners, and/or its employees and manager, for any injury, damage or death resulting from the negligence or other acts, howsoever organizations (if any), as a result of my activity/training/participation in "The Kodokan" training. You hereby release "The Kodokan," its owners/instructors/employees, any authorized agents, and any of its affiliated organizations from all guardians and legal representatives you now have, or may hereafter have from injury or damage resulting from your participation in the training/activities at "The Kodokan."
The Kodokan is a diverse and inclusive members club and our first appointment is to the safety and comfort of our members and guests.
We believe everyone is equal as human beings and we do not segregate our classes by gender, belt rank or any other type of category.
Training at The Kodokan will involve your full interaction with others of the opposite gender and those of diverse backgrounds. By checking the Release of Liability agreement box on your booking request form you agree to treat everyone with respect knowing that this is a full contact sport and are comfortable training with anyone and everyone alike.
You confirm that you are voluntarily participating in these activities/sport/training with knowledge that this is a full contact sport and will treat everyone with respect equally.
You acknowledge that you are comfortable training and interacting with any and everyone regardless of race or gender participating at The Kodokan, and do hereby verify this by ticking the box in the Release of Liability Form Agreement box on the previous page.
All participants understand that upon entering this facility ("The Kodokan"), their image may be recorded by various means, including but not limited to audio, video, and still photography. All participants release "The Kodokan" from any liability associated with the use of such images. All participants waive any right to the use or claim ownership of such images.
I have carefully read this agreement and fully understand its contents. I am aware that this release of liability is a contract between myself and "The Kodokan," its agents, and/or employees, and I confirm it of my own free will
